Who We Are
Resource Matching and Referral (RM&R) is a web-based application that streamlines patient referrals from acute care to post-acute care. It helps match patients with the most appropriate clinical programs and services, and facilitates care coordination through electronic referrals. Today, RM&R is used by over 80 healthcare providers across acute care, rehabilitation, continuing care, and home and community care sectors within the Toronto Central and CENTRAL regions of the GTA.
Challenges In Patient Referrals
A major challenge in the healthcare system is the prolonged occupancy of acute care beds by patients awaiting transfer to programs better suited to their needs. Traditionally, referrals have been slow due to manual processes such as paper forms and faxes, and limited awareness among healthcare providers of available programs and services.
Our Solution
RM&R addresses these challenges with an electronic referral system that simplifies and accelerates the referral process. Clinicians can send referrals digitally, allowing receiving organizations to quickly review, accept, decline, or request additional information. The system improves patient matching, identifies capacity constraints and service gaps, and enhances patient experience by providing a centralized, comprehensive repository of referral information for all participating healthcare providers.
